I found a flashback that shoes Hinako getting bit by a fox. There was a doctor’s note talking about foxes having rabies. Foxes are not naturally aggressive.
However, if they bite, they probably have rabies. Rabies got removed from Japan in 1957. However, Hinako got bit in the early 40s.
Rabies is a slow burn type of illness. It moves slowly up the nerves, until it attacks the brain. Once it gets to the brain, the person or animal acts aggressively.
Headaches
They also have a bit of a water phobia. In any case, her headaches are probably the rabies acting up. The pill numbs her to the pain, but it does not take care of the problem.
